Usefulness of Screening Breast Ultrasonography in Women with Negative Mammographic Results

Abstract
Purpose: To evaluate the usefulness of screening breast ultrasonography in asymptomatic women with negative mammographic results.

Materials and Methods: We included 681 patients with negative result on mammography from November 2005 to April 2007, and also had breast ultrasonography in our hospital. We categorized the ultrasonographic findings according to ACR BI-RADS, evaluated the biopsy rate and analyzed breast cancer cases. We also evaluated the means of ultrasonographic results according to breast parenchymal patterns on mammography.

Results: According to US BI-RADS guidelines: category 1 included 37%, category 2, 37.6%, category 3, 21.1%, category 4, 4.3%, and category 5, none. US-guided core needle biopsies were performed in 30 cases. Malignancy was one among them. The positive biopsy rate was 3.3% and the cancer detection rate was 0.15%. For the mammographic findings, the women with group 1 breast composition included category 1, 45% and category 2, 55%. In group 2, category 1 or 2 included 81.3%, category 3, 16%, and category 4, 2.7%. In group 3, category 1 or 2 included 70.3%, category 3, 23.5%, and category 4, 6.5%. In group 4, category 1 or 2 included 58.3%, category 3, 37.9%, and category 4, 3.9%.

Conclusion: Although we could find additional category 3 lesions(21.1%) and category 4(4.3%) in screening ultrasonography in asymptomatic women with negative mammographic results, the detection rate of breast cancer is 0.15%. Screening breast ultrasonography was useful in the detection of the lesion but it might raise false positive rate.
